# 3.14 Incident Reports

> When and how to file an incident report, including required details and where to submit.

  <Step title="Report every incident">
    Whenever an incident occurs while on duty, you MUST make a report. This can range from an altercation with an inmate, to noticing misconduct from another Corrections Officer.

  <Step title="Small incidents">
    For small incidents, you may use the `#incident-reports` channel in the DOC Discord.
  </Step>

  <Step title="Serious incidents">
    For serious incidents - ones regarding HUT or Life Inmates, or reporting another officer, please contact a member of High Command.
